Your patient has dry age-related macular degeneration (AMD)
No need to refer. Warn the patient that they are at risk of developing wet AMD and that this may cause them to lose a significant amount of central vision. They should check the vision in each eye on a daily basis and, more importantly, check for the presence of distortion. Either give the patient an Amsler chart or tell the patient to look at straight lines such as those on a window frame. If the lines are obviously not straight the patient may have wet AMD and should go back to their optician for assessment as soon as possible. Alternatively, you may refer the patient urgently to the local photodynamic therapy service for possible treatment.
